Encounter TEMPLATES

The note starts finished.

Nine years of static emergency medicine templates — the differential, the risk stratification, the disposition — now searchable, clickable, and ready to paste into any EHR.

The Chest Pain topic as Encounter Templates shows it: a note whose differential, risk stratification, orders, and disposition are clickable, with a HEART score calculator and a Copy button that produces plain text.

Find it. Make it yours. Paste it.

HOW IT WORKS
  1. 01

    Find the complaint

Search by name, by the alias you actually say, or by the dotphrase you already use — CP, chest pressure, and .chestpain all land on Chest Pain. Every topic is one click deep.

The library search box with “CP” typed in, and the one topic it matches.

    Make it yours

The differential, the orders, and the disposition are clickable, and the HEART score works out right in the note. Turn off what you didn’t consider, pick the path you took, and the sentence rewrites itself.

The differential opened to its option set: five diagnoses included, two turned off.

EDUCATION

The reading behind every sentence.

Each variant carries the teaching that justifies it — the HEART score cutoffs, the 0/1-hour troponin pathway, the fetal dose of a CTPA — cited to the guideline and kept out of the note.

Teaching is for you. The chart is for the patient. The two never mix in the paste.

CT PE in Pregnancy

ACOG Committee Opinion No. 723 supports that diagnostic imaging including CT should not be withheld from pregnant patients when clinically indicated. Fetal risk of anomalies, growth restriction, or abortion has not been reported with radiation exposure less than 50 mGy.5

Fetal radiation doses
StudyFetal Dose (mGy)
CT pulmonary angiogram0.01–0.66
VQ scan0.01–0.5
CT abdomen (low dose)~1.4
CT abdomen (high dose)1–25
CT pelvis (high dose)10–50

CTPA results in lower fetal radiation exposure compared to VQ scanning.5

FAQ

Questions, answered.

Is this tydotphrase?

Yes. Encounter Templates is the new home of tydotphrase: nine years of MDM templates and clinical education, same author, moved here and made searchable and editable. Old tydotphrase links will redirect to the new pages.

Do I put patient information into it?

No, and please don’t. You browse a library and copy text out; the site never sees a patient’s name, a chief complaint, or a note. That is by design — it is what keeps a free clinical reference free of patient data.

Does it work with Epic, Cerner, or Meditech?

It works with anything you can paste into. Copied text is plain text, with *** left exactly where your EHR expects a blank — Epic’s F2 walks through them as usual. Or fill the blanks on the page first and copy the finished note.

Do I need an account?

Not to look around: open the library and try a few templates. After a few clicks it asks you to sign in. The account is free, and it is where your own edited versions will live.

The library is open.

Free while it’s in beta. Sign in once and the whole library is yours to read, edit, and copy.
